Our mission is to support seniors and their families as they transition through complex medical situations, keeping them out of the hospital, the ER and rehab.

Our RN patient advocates support seniors and their families in the Triangle area of NC as they navigate complex medical situations. We are passionate about caring for our clients and making their aging process less stressful for them and their families.


Our proven program involves weekly home visits, medication management, accompanying our clients to their doctor visits and coordinating services.

After each visit to the home or the doctor we send a descriptive Family Report, containing our observations and thoughts, making sure everyone has all the information they need to make informed decisions. These reports are like having a second pair of eyes and ears and provide a picture of how our client is doing week-to-week.

Our company was founded in 2007 by Diane Halloran, RN, MPH after a 40-year career as a nurse. Diane saw the opportunity to keep our clients from suffering unnecessarily by offering the insight of a registered nurse to help people through life's changes.
